Patient Medical Intake & Research Consent Form
IMPORTANT! Each patient that will be utilizing the peptide protocols must first fill out and sign a medical intake and research consent form (CIF1003 form). Your clinic is responsible for making sure that each patient fills out this form prior to beginning their peptide treatment.
Once the form has been signed by you and your patient, please return it to Cellular Peptide via email for final submission to the IRB.
If you wish to dropship the peptide protocol(s) to your patient, please ensure that we have their completed 1003 form prior to placing the order.
